MilExTS®: Tactical Trauma Trainer teaches critical life-saving skills based on the Tactical Combat Casualty Care (TCCC) guidelines, specifically the MARCH sequence Massive hemorrhage, Airway, Respiration, Circulation, and Hypothermia. Through immersive scenario-based training, users learn how to apply tourniquets, manage junctional wounds, perform airway maneuvers, assess chest injuries, and recognize shock. These interventions are reinforced through guided instruction and independent decision-making in realistic, high-stakes simulations.

Beyond clinical interventions, MilExTS® develops the cognitive and operational skills essential for trauma care under battlefield conditions. Trainees are placed in dynamic environments that simulate combat pressure, requiring them to prioritize interventions, adapt quickly, and manage casualties with limited resources. The system emphasizes situational awareness, ethical decision-making, and team communication, including proper documentation using TCCC cards and relaying critical information to command or medevac units.

MilExTS® also promotes learning concepts such as repetition, realism, and self-paced mastery. Users can practice complex trauma scenarios multiple times in both guided and free-play modes, building muscle memory and confidence. With its portable XR delivery on standalone headsets, the system enables scalable, field-ready training without reliance on manikins, instructors, or traditional classroom infrastructure. These features make MilExTS® an ideal platform for preparing military and emergency responders to deliver effective care in any operational setting.

In MilExTS®, players are challenged to perform life-saving trauma care under battlefield pressure using the TCCC MARCH framework. Each scenario demands quick assessment, accurate intervention, and the ability to adapt to rapidly changing conditions.

Success is measured by how well and how quickly players manage injuries like massive hemorrhage, airway obstruction, and shock without instructor support. Real-time physiological feedback and after-action reviews reinforce decision-making and clinical skills.

The ultimate goal is to build confident, mission-ready responders capable of saving lives in austere or combat environments, with each session strengthening both medical competence and tactical performance.

MilExTS® is primarily intended for soldiers, military medics, combat lifesavers, and special operations personnel who require high-fidelity trauma training aligned with Tactical Combat Casualty Care (TCCC) standards. It also supports NATO forces, emergency medical services (EMS), and civilian first responders preparing for mass casualty and disaster scenarios. Designed for both individual and team-based use, MilExTS® enables trauma training in classrooms, simulation labs, and austere field environments. Its portability, realism, and interoperability make it ideal for soldiers and training commands, as well as medical educators and organizations integrating trauma care into Live, Virtual, and Constructive (LVC) simulation exercises.

MilExTS® assesses player performance through a combination of real-time physiological simulation, structured scoring metrics, and post-scenario analytics that reflect both clinical accuracy and operational effectiveness.

During each scenario, the system tracks:

1. Timing and sequence of interventions (e.g., how quickly and in what order the MARCH steps are performed).
2. Correct tool selection and application (e.g., proper use of tourniquets, airway maneuvers, or chest seals).
3. Response to changing casualty conditions (e.g., recognizing deterioration or shock and adjusting care accordingly).
4. Decision-making under stress, including whether life-threatening issues were addressed in time.

After the scenario, MilExTS® generates a detailed After-Action Review (AAR) highlighting key performance indicators like intervention accuracy, delays, missed steps, and patient outcome. In multiplayer or instructor-led settings, results can be reviewed collaboratively. When integrated with the RAPTOR analytics platform, training data can be aggregated to assess individual progress over time or unit-wide readiness across multiple sessions. This performance feedback loop supports targeted improvement, certification readiness, and operational preparedness.
